Lead Belong Health’s virtual and home-based medical group, driving clinical excellence, risk adjustment performance, and Stars quality outcomes across MA and SNP populations. Oversee physicians, APPs, and clinical staff while building workflows that close gaps in care, improve member experience, and strengthen value-based care performance. Develop programs for accurate HCC documentation, HEDIS gap closure, and high-touch, patient-centered care delivery. Build and maintain deep relationships with regional physician networks and health systems to enhance coordination, referrals, and VBC success. Partner with operations, product, and technology teams to scale innovative care models across markets.
